Register with us as a New Patient

How to Register

We are happy to register new NHS patients within our practice boundary area.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes.  All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

Register online

Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Please complete one form for each family member.

Safe Surgeries

We are a Safe Surgery for everyone in our practice area.

We might ask for ID or proof of address. But if you don’t have any and you live in our practice area, you can still register with us.

We won’t ask for immigration documents.

If you would rather submit your application to register personally please download and complete the forms below.

A registration form AND a Questionnaire need to be completed for each adult and child in the household who wishes to register with the practice.

It is preferable that we see proof of identification and proof of address. These should be taken to reception with the registration form and questionnaire.

We ask that all members of a family living at the same address be registered with us.

Please be aware that you will be registering with the Practice rather than an individual GP.

Organ Donation

From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
