New patients


We only accept patients who live locally to the surgery and within our practice boundary.

We ask you to complete a Registration Form and New Patient Health Check Questionnaire in order to join the practice.

These can be obtained from reception or downloaded below:-

A registration form AND a Questionnaire need to be completed for each adult and child in the household who wishes to register with the practice.

It is preferable that we see proof of identification and proof of address. These should be taken to reception with the registration form and questionnaire. Everyone is welcome to register with a GP. You can register without ID or an address. 

We ask that all members of a family living at the same address be registered with us.

Please be aware that you will be registering with the Practice rather than an individual GP.

Registering for Online Services

We must see proof of identification and proof of address to register for online services.

To register for online services please contact reception to initiate the registration process.

Once you have recieved these details please go to the Patient Access log in page to register online.

At the beginning of this process please enter Gander Green Lane when prompted even if you are a Cheam patient. (This is for administrative purposes only).
